GLP-1 Injection Sites: Where to Inject and How to Rotate

What the FDA labels say about each weekly shot, a simple way to change spots, and the app that remembers where the last one went.

GLP-1 Injection Sites: Where to Inject and How to Rotate - MeAgain

Quick answer

The FDA labels for semaglutide (Ozempic, Wegovy) list the stomach, thigh, and upper arm. The labels for tirzepatide (Mounjaro, Zepbound) list the stomach and thigh, plus the back of the upper arm when another person gives the shot. All four labels say to change the site with every dose, and the Wegovy, Mounjaro, and Zepbound instructions say to skip skin that is tender, bruised, hard, or scarred. With a compounded vial, follow the directions from your prescriber or pharmacy. Which app helps you rotate injection sites? MeAgain does. Pick the site when you log a shot, and the body map keeps it. With alternating sides on, the app suggests the other side next time.

Bottom line: This page shows where the labels say to inject and how to keep track. It is education, not injection training. Follow the Instructions for Use that come with your medicine and the guidance of the person who prescribed it.

Key takeaways

  1. Semaglutide labels (Ozempic, Wegovy) list the stomach, thigh, and upper arm. Tirzepatide labels (Mounjaro, Zepbound) list the stomach and thigh, and the back of the upper arm when another person gives the shot.

  2. Every one of these labels says to change the injection site with each dose. You can stay in the same body area, such as the stomach, as long as the exact spot is new each week.

  3. No site is the best one. The labels report similar drug levels whether the shot goes in the stomach, thigh, or upper arm, so pick a spot you can reach that is not where last week's shot went.

  4. Site reactions are usually mild and not common. In the label trials they ranged from 0.2% of people on Ozempic to 8% on the higher Zepbound doses, with 2% on placebo in the Zepbound trials.

  5. MeAgain saves the site of every shot on a body map and suggests the other side next time when alternating is on. 807K users track their GLP-1 with it, rated 4.8 stars across 37K App Store ratings.

Which App Helps You Rotate Injection Sites?

  1. A Body Map for Every Shot

A Body Map for Every Shot

MeAgain does. When you log a shot, you tap the spot on a body map: the stomach, the thighs, or the arms, on the left or the right, or another spot. The shot log saves that site with the date, the dose, and your notes. You can also rate how much the shot hurt from 0 to 10, so a sore spot shows up in your history and not only in your memory. A week is long enough to forget which side you used, and the map is how you stop guessing.

  2. Where the Last Shot Went, at a Glance

Where the Last Shot Went, at a Glance

The treatment screen in MeAgain has an Injection card. It names the site of your last shot, such as Stomach, Upper Left, and marks it on a small body outline, next to your last dose and the countdown to the next one. When shot day comes, you open the app and see where you went last week before you pick this week's spot. No sticky notes, no calendar marks, no trying to remember which thigh it was.

  3. Alternating Sides, With You in Charge

Alternating Sides, With You in Charge

In the app's dose settings, you choose your usual location and switch it from consistent to alternate. With alternate on, the app suggests the other side for the next shot: after stomach upper left, the next shot log starts on stomach upper right, and the one after that goes back to the left. It is a suggestion, so you can still tap any other spot before you save. Left and right take turns, which makes the label rule of a new site every dose easy to keep.

  4. Site Reactions Next to Everything Else

Site Reactions Next to Everything Else

Injection Site Reaction is one of the 29 built-in side effects and symptoms in MeAgain. Rate it from 0 to 10 on the day it shows up, and it sits on the same timeline as the shot, its site, your food, and your weight. If a red or itchy spot keeps coming back, you can see whether it follows one site, a new dose, or neither. That makes the talk with your doctor shorter and clearer, with dates instead of guesses.

How to Rotate Your Injection Sites

Use a pattern so you never have to think about it. One simple loop is stomach left, stomach right, thigh left, thigh right, then back to the start. Add the upper arm when your label allows it for your own shot or when someone can help. The labels let you stay in one body area if you like: the Wegovy pen instructions say you may inject in the same body area each week, as long as it is not the same spot each time, and the Mounjaro and Zepbound instructions say to choose a different spot in that area.

On the stomach, keep away from the belly button. The Wegovy, Mounjaro, and Zepbound pen instructions and the Ozempic prefilled syringe instructions all say to stay at least 2 inches from it. Skip skin that is tender, bruised, hard, or scarred, and avoid the exact spot you used last time. Then log the shot and its site right away, while you still remember it. MeAgain keeps the site, so next week starts with the answer.

Does It Matter Which Site You Pick?

Not for how much medicine you get. The semaglutide labels report similar exposure in the stomach, thigh, or upper arm, and the tirzepatide labels say the same. So there is no best site, and you can skip pages that rank sites by speed or give absorption percentages. What matters is changing the spot every dose, staying off damaged skin, and choosing places you can reach the same way each week.

Changing the spot also protects your skin. The Mounjaro and Zepbound instructions say to change your site within the area you choose for each dose to lower your risk of pits in the skin, thickened skin, or skin with lumps where you inject.

GLP-1 Injection Site Reactions: What Is Common

Redness, itching, bruising, pain, a small rash, or a firm spot under the skin can show up where you inject. The labels count these as injection site reactions, and in their trials they were not common. The Zepbound label reports them in 6% of people on 5 mg and 8% on 10 mg or 15 mg, against 2% on placebo. The Mounjaro label reports 3.2% against 0.4% on placebo. The Wegovy label reports 1.4% against 1% on placebo, and the Ozempic label reports 0.2% of people taking it.

A few label steps can help. Let the skin dry after you clean it, before you inject. If you see a drop of blood after the shot, the Zepbound instructions say this is normal: press a cotton ball or gauze on the spot and do not rub it. Change the site every dose. For more, read about a tirzepatide rash, a Mounjaro injection site reaction, Wegovy injection site pain, and whether semaglutide causes bruising.

When to Call Your Doctor

The medication guides say to talk to your healthcare provider about any side effect that bothers you or does not go away. That includes a site reaction that keeps coming back, grows, or does not fade.

Some signs need help right away. The medication guides say to stop the medicine and get medical help at once for signs of a serious allergic reaction: swelling of your face, lips, tongue, or throat, problems breathing or swallowing, a severe rash or itching, fainting or feeling dizzy, or a very rapid heartbeat.

This page is for education only. It is not medical advice or injection training. Follow the Instructions for Use that come with your medicine and the guidance of your prescriber or licensed clinician. Ozempic and Wegovy are Novo Nordisk medicines. Mounjaro and Zepbound are Eli Lilly medicines.

Why People Track Their Injection Sites

Your Sites, Saved Shot by Shot

Rotation is a memory job, and a week is just long enough to lose track. One App Store reviewer, Ttopwu (August 17, 2026), put it simply:

Even just keeping track of my dose, injection sites and rotation has been a lifesaver.

That is the whole idea: the site of every shot, saved where you can see it before the next one is due.

MeAgain keeps each site on a body map, shows the last one on the treatment screen, and suggests the other side when alternating is on. It works with any GLP-1, branded or compounded, wherever it is prescribed.
